What you'll do 

As a Social Worker at TDHS, you will work as part of our interdisciplinary team to provide high quality, person centred social work services to patients, clients and their families. You will undertake psychosocial assessments, provide counselling and support, assist with discharge planning, and connect people with appropriate services and resources.

You will work closely with patients, families, colleagues and external service providers to support coordinated care and positive outcomes. The role also involves providing education, consultation and referrals, contributing to clinical care planning, supporting the development of group programs where appropriate, and participating in quality improvement activities across the service.

This is a fixed term, part-time maternity leave position working 16 hours per fortnight at Grade 2 classification. Terms and conditions of employment are in accordance with the Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021–2026.

About You

You will be a qualified Social Worker with a Bachelor of Social Work or other relevant tertiary qualification and be eligible for membership or registration with the Australian Association of Social Workers (AASW). You will bring experience working within a hospital, community health service or similar healthcare environment, together with strong clinical skills in psychosocial assessment, risk assessment, complex care planning, discharge planning and social work interventions.

You will have a sound understanding of the Social Work role across acute care, rehabilitation, palliative care, aged care and community health services, as well as the theoretical frameworks that guide contemporary Social Work practice. You will be confident working both independently and as part of a multidisciplinary team, with excellent communication and interpersonal skills and the ability to build effective relationships with patients, families, colleagues and external service providers.

Strong organisational, IT and Microsoft Office Skills are desirable.
