What you’ll do
As Community Care Manager, you will lead the day-to-day operations of TDHS Community Care services, ensuring safe, high-quality, person-centred care that meets all funding, legislative and regulatory requirements. You will coordinate care, oversee electronic health records and statutory reporting (including Community TrakCare), manage budgets and routine expenditure, and ensure services are delivered efficiently and in line with organisational policy. You will be a key contact for clients, carers, staff, contractors and partners, resolving issues and ensuring a seamless service experience.
You will lead governance, compliance, audit readiness and risk management for the program, driving continuous improvement and service reform in response to sector change. You will also build and support a high-performing team through workforce planning, recruitment, performance development and training, while working closely with the Director of Community Health, Executive and external stakeholders to identify risks, strengthen service quality and shape the future of community care at TDHS.

About You
You are an experienced leader in community or aged care with strong knowledge of the Aged Care Act, Aged Care Quality Standards, CHSP, Home Care Packages and HACC. You bring proven experience in program management, budgeting, compliance, audit readiness and workforce leadership, along with advanced communication and digital skills. Above all, you are values-driven, adaptable and committed to delivering high-quality care that makes a genuine difference to the community.
