About the Role

The Chronic Disease Nurse Practitioner is dedicated to delivering tailored, person-centred care to individuals who are either at risk of developing or have been diagnosed with complex and chronic health conditions, ensuring that Timboon and District Healthcare Service consistently provides exceptional service.

 

In this vital role, you will specialise in supporting consumers living with chronic conditions by:

• Coordinating individualised care plans.

• Providing chronic disease education and monitoring.

• Detecting early signs of deterioration and facilitating timely escalation of care.

• Collaborating closely with external stakeholders to ensure seamless integration of service

 

Your responsibilities include conducting home visits and in-person consultations to enhance the quality of life for consumers with chronic conditions. You will focus on improving their understanding of their health, promoting medication adherence, and reducing preventable hospital admissions.

 

Qualifications, Skills and Experience

To be successful in this role you will have:

 

  • Registration as a Division 1 Registered Nurse with AHPRA
  • Nurse Practitioner – Masters level qualification with AHPRA endorsement to practice as a Nurse Practitioner
  • Demonstrate evidence of provision and of coordination of comprehensive contemporary person-centred care through advanced practice skills
  • Demonstrate advanced knowledge and application of continuous quality improvement principles in improving consumer outcomes
  • Demonstrate advanced interpersonal and problem- solving skills
  • Proactively develop self and others through knowledge and application of the lifelong learning framework
  • Demonstrate evidence-based practice, conduct research activities and quality initiatives to improve and inform practice change
  • Demonstrate evidence of leading innovation to address emerging needs in healthcare provision
  • Demonstrate advanced and expanded clinical knowledge and skills in the speciality field of chronic disease nursing
  • Demonstrate safe, quality, cost effective care and practice within the Nurse Practitioner’s approved scope of practice that is consistent with contemporary standards and evidence-based practice
  • Experience in developing structures, systems, and processes to ensure quality assurance and efficient service implementation
  • Experience in collecting and analysing service data; reporting variances and benchmarking to align practice service with best practice.
  • Proven organisational and time management skills with the ability to prioritise and manage a diverse caseload
  • Ability to develop and manage consumer care plans in conjunction with a multidisciplinary team
  • Excellent communication skills – written and verbal
  • Demonstrate a commitment to ongoing educational and professional development across all levels of the nursing profession including management
  • Demonstrate analytical skills through research findings to enrich and update delivery of health care and community awareness programs

 

All successful applicants are required to satisfactorily clear a National Police Check (conducted by TDHS) and a National Disability Insurance Scheme (NDIS) Check and Working with Children Check (provided by you). All successful applicants will be required to obtain the annual influenza.
