Great Ocean Road Health serves the communities of Lorne, Apollo Bay, and the Surf Coast, providing high-quality, accessible healthcare close to home. Formed in 2019 through the integration of Lorne Community Hospital and Otway Health, we offer a range of services across our Lorne and Apollo Bay campuses. Our goal is to meet the growing healthcare needs of the community, ensuring sustainable, robust care for the future while maintaining local services and staff levels.
About the Role
The Diabetes Educator provides high quality diabetes education, clinical management, and information and support to clients and staff. The role includes primary health care focused assessment, planning, early detection, referral, monitoring and maintenance and direct care to individuals, families and groups in relation to diabetes. The Diabetes Educator will also take an active role in the coordination and provision of health promotion activities.
Essential Education:
- Bachelor’s degree or higher in a health related field and completed a post graduate degree in diabetes education
- Credentialled or working towards accreditation by the Australian Diabetes Educators Association
- Current NDIS workers check
Desirable skills:
- Self motivated and able to work without supervision
- Ability to act in an advisory role to management
- Proficiency in group therapy programs
