About the role
SOCIAL WORK - Grade 1 or 2
Up to 38 hours per week from December 2025 until November 2026, a backfill of parental leave. (Applicants can apply for all or part of hours).
Colac Area Health is a values-based organisation providing a unique integrated health service comprising of over 500 employees, providing Acute Care, Aged Care, Community and Allied Health Services.
The Social/Welfare Work Department of Colac Area Health consists of a team of social workers who provide a range of services that include psycho-social assessment, complex care planning, grief and bereavement intervention, counselling, advocacy, mediation, consultation and liaison for patients/clients/residents and their families/carers across a variety of settings and service delivery types. Services include acute, outpatient paediatric service & community palliative care.
This position offers the opportunity to work as part of an effective interdisciplinary team, providing quality care to clients through use of clinical skills and interventions within the scope of Colac Area Health’s integrated services. The role gives exposure to a rural and regional healthcare service, partnering with larger health networks including Barwon Health, to ensure the role is supportive and fosters career growth across a range of areas.
This position is open to both Grade 1 and Grade 2 applicants with the successful applicant being graded as per their relevant experience. Whilst the position is for up to 38 hours, we are open to a part-time shared position for appropriate candidates.
What’s required?
Grade 1
Essential:
- Bachelor or Master of Social Work degree and eligible for membership with the Australian Association of Social Work.
- Proficient clinical skills in undertaking comprehensive psychosocial assessments, clinical reasoning, and formulating appropriately targeted treatment plans/ interventions with achievable goals for clients in a variety of settings.
- Excellent communication skills and a proven ability to navigate solutions towards desired outcomes.
- Ability to work both autonomously and within multidisciplinary teams to achieve desired patient outcomes with a commitment to person centred practice.
- Ability to maintain high level professional practice standards at all times.
- Current Working with Children Check, Police Check, and Current Victorian Driver’s Licence.
Grade 2
Essential:
- Bachelor or Master of Social Work degree and eligible for membership with the Australian Association of Social Work.
- Advanced clinical skills in undertaking comprehensive psychosocial assessments, clinical reasoning, and formulating appropriately targeted treatment plans/ interventions with achievable goals for clients in a variety of settings.
- Excellent communication skills and a proven ability to lead solutions towards desired outcomes.
- Ability to work both autonomously and within multidisciplinary teams, providing seniority and expertise as appropriate, to achieve desired patient outcomes with a commitment to person centred practice.
- Ability to maintain high level professional practice standards at all times.
- Ability to proactively contribute to and adapt to internal and external change.
- Current Working with Children Check, Police Check, and Current Victorian Driver’s Licence.
Desirable:
- Experience supervising Grade 1 clinicians, Allied Health Assistants and/or students
- Experience managing a varied and complex caseload.
- Demonstrated clinical expertise in paediatric developmental assessment, care coordination or case management.
