About the role

 

SOCIAL SUPPORT FACILITATOR
Temporary Part Time - 24 Hours per week until 14/03/2027

(maternity leave relief for 12 months)

 

Colac Area Health is a values-based organisation providing a unique integrated health service comprising of over 500 employees, providing Acute Care, Aged Care, Community and Allied Health Services.

 

This role will provide a wonderful opportunity to expand our current innovative Social Support team that generate and conduct one to one and group programs for carer respite, social connections and community engagement within a friendly and safe environment. 
Social Support engaging programs include exercise, outings, events, educational sessions, carer respite, volunteer visiting and are sportive of participants to maintain and improve their wellbeing and skills.

 

What’s required? 

 

  • Minimum Cert III in Individual Support or its equivalent or working towards.
  • First Aid Certificate Level 2 or willingness to undertake.
  • Demonstrated rapport with older people and an understanding of their needs.
  • Experience of working with the aged as well as with participants with intellectual and/or physical disabilities.
  • Demonstrated understanding of the needs of aged people and the social experiences that will add value to their lives.
  • Knowledge of social support and other relevant services
  • Excellent personal and communication skills.
  • Excellent planning and organisational skills, including time management.
  • Knowledge of and experience with computer systems including MS Word, Publisher, Excel, Outlook and social media platforms.
  • Capacity to work as a sole worker and co-operatively within a team.
  • A positive, energised and encouraging outlook.
  • Experience of delivering a person-centred approach.
  • Experience with group work and managing group dynamics.
  • Current Driver’s Licence and Working with Children Check.
