About the role

 

Pharmacist - Regional and Specialist Services Support  

Full Time

 

Colac Area Health is a values-based organisation providing a unique integrated health service comprising of over 500 employees, providing Acute Care, Aged Care, Community and Allied Health Services.

 

In this role working in conjunction with the Director of Pharmacy, you will be responsible for the provision of Pharmacy Services within Colac Area Health to inpatients and organisational departments and clinical and Medication Safety governance support to services within the Polwarth Partnership.

 

 

What’s required? 

  • Registered as a pharmacist with AHPRA
  • Member of Advanced Pharmacy Australia (AdPha) or international equivalent
  • Minimum of 4 years post registration hospital pharmacy experience, with no less than 2 years as a Grade 2 hospital pharmacist (or equivalent)
  • Completion of postgraduate studies in Clinical Pharmacy, Pharmacy Practice or equivalent or working toward same
  • Recognised qualifications and/or experience in delivering pharmacy services and support to Emergency or urgent care department, Day Oncology, remote clinical pharmacy activities and Peri Operative services
  • Credentialed in the role of Partnered Pharmacist Medication Charting (PPMC)
  • Demonstrated ability to apply and share expert clinical knowledge and provide patient focused care
  • Excellent written and verbal communication skills
  • Well-developed problem solving, analytical and time management skills
  • Demonstrated team leadership experience including multidisciplinary teams
  • Demonstrated involvement in and ongoing commitment to research and quality improvement projects
  • Experience and proficiency using various software and IT systems such as email, Microsoft Suite and pharmacy specific IT systems/programs
