About the role
Pharmacist Grade 2 – Subacute Programs (Outreach, Outpatient Programs and Aged Care Support/Intake)
12 hours per week
Colac Area Health is a values-based organisation providing a unique integrated health service comprising of over 500 employees, providing Acute Care, Aged Care, Community and Allied Health Services.
In this role working in conjunction with the Director of Pharmacy, you will be responsible for the provision of safe and appropriate consumer centred Medication assessment and advice in subacute, aged care and outpatient settings. The role will liaise with other health care professionals including referral of complex clients to appropriate services.
What’s required?
- Registered as a pharmacist with AHPRA,
- Demonstrated experience in a community setting as a community pharmacist with a minimum of 4 years’ experience.
- Experience supplying or supporting Aged Care Pharmacy Services.
- High level verbal and written communication skills.
- High level interpersonal and professional organisational skills.
- High level of clinical competence; skill and knowledge.
- Competent computer literacy skills.
- Ability to work autonomously but be both competent and confident working with a broad range of health care professionals.
- Demonstrates knowledge and understanding of government legislation, policies and procedures, risk management and OH&S relative to Community Services within the public sector.
- Current Victorian’s Drivers Licence.
- Current Police Check.
