HITHPAC Home Nurse - Registered Nurse

The HITH/PAC Home Nurse role will be responsible for assessing, planning, delivery and evaluating safe and appropriate consumer centred nursing care in the community setting. The nurse can assess, treat and escalate in the case of deterioration appropriately while managing the care of multiple clients independently, working collaboratively with the Home Nursing team in the provision of complex care in the community setting.  The nurse ensures that the delivery of quality, evidence based best practice of care is delivered, while being a leader and mentor to junior staff and students within the Home Nursing team. 

The HITH/PAC Home Nurse will act as a clinical resource within the team and provide clinical expertise and problem-solving ability for service provision.

• Registered Nurse Division 1 holding current registration with the AHPRA (Australian Health Practitioners Regulation Agency), with a minimum of 2 year post qualification clinical experience 

• Basic Life Support, Anaphylaxis, CVAD, Venepuncture and IV cannulation competencies 

• Demonstrated competency in clinical skills relevant to home nursing, including HITH/PAC clinical care, complex wound management, continence and diabetes management and palliative care 

• Ability to perform comprehensive clinical assessments and develop, implement, and evaluate goal-directed care plans 

• Proficient in safe medication management and complex care procedures 

• Ability to practice independently, set priorities, and manage a clinical workload in a home setting, while also contributing to the team 

• Highly developed verbal and written communication skills for patient education, documentation, and liaison with stakeholders 

• Demonstrated commitment to patient-centred care, quality improvement, and safety standards 

• Understanding of medico-legal documentation compliance and competence in using electronic documentation systems (TRAK) for patient records and data collection 

• High level interpersonal and professional organisational skills 

• Knowledge and understanding of government legislation, policies and procedures risk management and OH&S relative to Home Nursing within the Public sector 

• Current Victorian Drivers Licence, Working with Children Check, and clear Police Check
