About the role
Hospital in the Home/Post Acute Care Coordinator
16 hours per fortnight permanent
Our Hospital in the Home (HITH) service aims to provide high quality care for patients in the comfort of their own home, providing them with the care they would otherwise receive in hospital. Patients remain under the care of the doctor they were admitted under and are seen by nurses who deliver their care needs. Frequency of visits depend on the patients care needs. Nurses remain in constant communication with the patients’ medical team.
The Post Acute Care (PAC) program aims to enable a safe and timely discharge from hospital. The program ensures that the patient returns home with a suitable package of community based supports. Patients can be linked with ongoing long term support in the community if required. Services arranged by PAC are provided for an agreed term period following discharge from CAH.
This role will be responsible for the effective management and coordination of a client’s care in the post acute phase and acute phase when referred to the HITH or PAC programs. As well as assessing program eligibility of the client, service coordination, client assessment, follow up and evaluation. This position also requires effective financial management, program promotion, evaluation of the programs and overseeing home nursing staff in the delivery of care to HITH and PAC clients.
What's Required?
- AHPRA Registered Nurse, Division 1 with minimum 3 years post graduate experience.
- Ability to articulate HITH/PAC objectives.
- Experience working with community service providers, Allied Health and GP’s.
- Preparedness to participate in ongoing staff development to support the role.
- Ability to work autonomously.
- Advanced assessment skills and relevant experience with disease management.
- Current drivers licence.
- Current Working with Children check.
- Competent computer literacy skills.
