About the role 

We are seeking a motivated Team Leader to coordinate the Barwon South West Assessment Team and support the delivery of high-quality aged care assessment services across the region.

 

This important supervisory role is responsible for overseeing day-to-day team operations, coordinating workloads, maintaining effective work practices and ensuring compliance with Victorian Department of Health KPIs, Barwon Health policies and relevant program guidelines.

 

Working within the My Aged Care framework, including the Aged Care Act 2024 and Commonwealth Home Support Program (CHSP) requirements, you will help deliver a nationally consistent screening and assessment process for people seeking access to CHSP services and other aged care supports. The role also supports consumers to make informed choices about the services and supports that best meet their individual needs.

 

This is an excellent opportunity for an experienced health professional with strong leadership and coordination skills to contribute to positive outcomes for older people and their carers across the Barwon South West region.

 

Key Responsibilities 

  • Provide day-to-day leadership, supervision and operational coordination of the Barwon South West Assessment Team to ensure services meet My Aged Care, Barwon Health and Victorian Department of Health requirements and KPIs
  • Deliver high-quality, consumer-centred screening and assessment using the Integrated Assessment Tool (IAT), develop support plans and coordinate appropriate referrals and short-term case management for vulnerable consumers
  • Support quality improvement through staff education and clinical support, monitoring documentation and assessment practices, resolving operational issues and contributing to reporting, audits and service improvement initiatives

 

What we’re looking for 

  • Tertiary qualification in a relevant health discipline with current registration or accreditation, a Victorian driver’s licence and NDIS Clearance
  • Demonstrated knowledge of My Aged Care, Single Assessment Service, CHSP, NDIS and community or home-based aged care services
  • Strong organisational, communication and problem-solving skills, with the ability to work independently and collaboratively within a multidisciplinary team and manage competing priorities effectively
