About the facility
Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
Social Workers at Barwon Health work across a diverse range of the Health areas including Acute, sub-acute and community based program areas at a number of campuses of Barwon Health. They require clinical knowledge and skills relevant to working as a clinician in a health setting. The social work clinician will have an awareness and understanding of the health service environment. The roles are based in a multidisciplinary/team setting. Some parts of our service work across a 7-day a week roster, while others Monday to Friday. All roles can work across both the University Hospital of Geelong (UHG) Campus & the McKellar Inpatient Rehabilitation (IRC) campus. The work is team based and is dynamic and rewarding.
About the role
Our Social work roles are part of a Social Work team and department, whom work closely together and provide daily peer support. The daily clinical work is also team based within a multi-disciplinary setting (i.e. with nurses, Drs and other Allied health). This role is based at University Hospital Geelong across a work roster 7 days of the week (i.e. Sat or Sun, with day/s off during the week). Weekend work renumerated at time and a half and public holidays double time. An extra week’s annual leave is an entitlement if you work across 10 or more weekends a year.
Position dimensions:
Without referral to Manager (RESPONSIBLE)
- Provide psychosocial assessment involving the patient, family and engaged agencies.
- Work alongside treating team, providing consultation.
- Provide clinical intervention ensuring that the needs of the client and family are attended to.
- Refer patients to appropriate Barwon Health departments or external agencies as appropriate.
- Maintain appropriate records as required by Barwon Health and statutory authority
- It is desirable that you have knowledge of VCAT, NDIS, Aged Care processes and health
Other:
- Participate in wider service meetings, education opportunities and working groups.
- Management of complex risk situations.
- Be part of a dynamic and supportive social work and multidisciplinary team.
