About the role 

As the successful applicant you will work independently to provide Subacute at Home patients with high quality, flexible assessment, responsive therapies and care in the patient's homes, it is a challenging role, expecting you to work both autonomously but also contribute to a dynamic, highly skilled multidisciplinary team for this 7 day service.

 

 

Key responsibilities 

  • Provide evidence-based nursing care in accordance with ANMAC competency standards, Barwon Health policies, and best practice principles, ensuring coordinated, goal-directed care that supports the health and wellbeing of patients and carers.
  • Maintain compliance with National Safety and Quality Health Service (NSQHS) Standards, participate in quality improvement and audit activities, apply effective communication frameworks (AIDET and ISBAR), and contribute to a safe, culturally responsive care environment.
  • Work effectively within multidisciplinary teams to coordinate care, foster positive working relationships, and contribute to a collaborative and supportive team culture that delivers optimal patient outcomes.
  • Demonstrate ongoing professional development through CPD, maintain AHPRA registration and scope of practice requirements, ensure accurate clinical documentation, and comply with organisational policies, information management, and occupational health and safety requirements.

 

What we’re looking for 

  • Current AHPRA registration as a Registered Nurse with demonstrated clinical experience in delivering evidence-based, person-centred care.
  • Demonstrated ability to work collaboratively within multidisciplinary teams, supported by excellent communication and interpersonal skills.
  • Strong clinical assessment, care planning and problem-solving skills, with a commitment to quality improvement and patient safety.
  • Well-developed organisational and digital literacy skills, with the ability to maintain accurate documentation and a commitment to ongoing professional development and mandatory compliance.
