About the Role

We are seeking compassionate, motivated and patient-focused Registered Nurses to join our Geriatric Evaluation and Management (GEM) Rehabilitation Ward.

This is an excellent opportunity for a Registered Nurse who is passionate about providing high-quality care to older people and supporting patients to regain their independence, confidence and quality of life following illness, injury or hospitalisation.

As a Registered Nurse, you will provide high-quality, person-centred nursing care while working closely with patients, families and the multidisciplinary team.

Key responsibilities

• Providing safe, evidence-based and person-centred nursing care.
• Completing comprehensive nursing assessments and developing individualised care plans.
• Monitoring patients' clinical condition and responding appropriately to changes in health status.
• Supporting patients with rehabilitation, mobility, activities of daily living and reablement goals.
• Working collaboratively with physiotherapists, occupational therapists, allied health professionals, medical staff and other members of the multidisciplinary team.
• Supporting medication management and other clinical interventions.
• Educating patients and families to promote self-management and independence.
• Contributing to effective discharge planning and transition of care.
• Maintaining accurate and timely clinical documentation.
• Participating in quality improvement, education and clinical governance activities.
• Promoting dignity, respect, choice and positive patient experiences.

• Current AHPRA registration as a Registered Nurse.
• Strong clinical assessment and nursing skills.
• Excellent communication and interpersonal skills.
• A commitment to person-centred and evidence-based care.
• The ability to work effectively within a multidisciplinary team.
• A positive, flexible and collaborative approach.
• A genuine interest in supporting older patients to achieve their rehabilitation goals.

About the GEM Rehabilitation Ward

Our GEM rehabilitation service provides specialised multidisciplinary care to older patients with complex medical, functional and rehabilitation needs.

Working as part of a collaborative multidisciplinary team, you will support patients throughout their rehabilitation journey, with a strong focus on reablement, independence, mobility and safe discharge planning.
