About the role
The Post Acute Care Coordinator (Nurse Unit Manager Level 2) will provide leadership in the planning, implementation, and coordination of care aligned with the Post Acute Model of Care. The role ensures the team delivers high-quality, consumer-centred care and achieves excellent health outcomes for people recently discharged from emergency, acute, and sub-acute services who require short-term, community-based support to recuperate safely at home.
Working within a multidisciplinary environment, you will support the coordination of daily operations, contribute to service development, and provide clinical and operational leadership to the Post Acute Care team. This role plays a key part in strengthening community-based care pathways and improving patient flow across the health system.
Key responsibilities
- Lead the planning, implementation and coordination of care in alignment with the Post Acute Model of Care.
- Provide clinical leadership, supervision and support to Post Acute Care staff.
- Oversee daily operational management of the team, ensuring safe, efficient and high-quality service delivery.
- Contribute to quality improvement initiatives, service development and change management activities.
- Support achievement of program objectives and key performance indicators.
- Foster a positive, collaborative and learning-focused team culture.
What we’re looking for
- Registered Nurse with a minimum of 5 years’ clinical experience and at least 3 years’ management experience.
- Proven leadership experience in a complex health environment, including service coordination and operational oversight.
- Strong communication, interpersonal and conflict resolution skills.
- Demonstrated ability to lead teams, drive change and deliver negotiated outcomes.
- Sound analytical skills with the ability to interpret data to support service improvement.
