Paediatric Nurse, Hospital In The Home / Daystay

Key responsibilities 

• Provide nursing assessments and clinical care to HITH/PAC and daystay patients as per care plan
• Liaise with medical team/HITH coordinator/Ambulatory Manager with clinical concerns
• Escalate care in the community to ensure safety of patients
• Undertake home visiting risk assessments
• Liaise with HITH coordinator to discuss changes in care plan
• Assist HITH coordinator with intake and coordination
• Actively communicate with internal and external stakeholders 

What we’re looking for 

• Current registration as a registered nurse with Nursing and Midwifery Board of Australia (NMBA) administered through the Australian Health Practitioner Regulation Agency (APHRA)
• Experience in acute paediatric or paediatric community care
• Current Victorian drivers licence
• Extensive clinical skills and knowledge, and able to work autonomously
• Sound clinical assessment and judgement
