About the role
The Mental Health Hospital in the Home (MH-HITH) program is a key reform initiative arising from the Royal Commission into Victoria’s Mental Health System, designed to provide intensive, recovery-oriented care as an alternative to inpatient admission.
This is a unique opportunity for an experienced Nurse Practitioner to work at the forefront of community-based acute care, providing clinical leadership during evenings and weekends, when it is needed most.
The MH-HITH service delivers:
- 9 bed-equivalent packages of care for adults and young people (16+)
- 7-day operation (0700–2130 hrs)
- Intensive treatment for individuals with high acuity mental health and/or AOD needs
- A strong focus on recovery, safety, and reducing hospital admissions
You will be part of a dynamic, multidisciplinary team committed to delivering flexible, person-centred care in the least restrictive environment.
Key Responsibilities
- Provide advanced clinical assessment, diagnosis, and treatment in community settings
- Deliver autonomous, evidence-based care for high-acuity consumers
- Offer clinical leadership and decision-making support outside standard business hours
- Lead risk assessment and crisis intervention in complex presentations
- Collaborate with multidisciplinary teams to ensure safe, coordinated care pathways
- Contribute to service development and model evaluation as part of a state-wide reform initiative
What We Are Looking For
To succeed in this role, you will bring:
- Endorsed Nurse Practitioner registration (AHPRA)
- Postgraduate qualifications in Mental Health Nursing
- Demonstrated experience in acute mental health or crisis services
- Strong capability in autonomous clinical decision-making and risk management
- A commitment to recovery-oriented and trauma-informed care
- Current Australian Driver’s Licence
