About the role
Residential In-Reach (RIR) provides an alternative to the emergency department where clinically appropriate and safe, for people living in residential aged care facilities.
Become part of a specialist multi-disciplinary team responsible for assessment and management of resident’s acute medical condition within the aged care facility in collaboration with the resident and their representatives to avoid unnecessary hospital presentations.
Supported by Geriatricians, Registrar, General Practitioner, Nurse Practitioner and Registered Nurses this is a unique role that provides specialist care to residents of residential aged care facilities.
Key responsibilities
- Triage and comprehensive assessment of deteriorating residents
- Comprehensive advanced nursing care
- Care planning and delivery
- Follow up consultations
- Capacity building and education of residents, staff and families
- Communication of outcomes and discharge planning
What we’re looking for
- Authorised to practice as a Nurse Practitioner and Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA) together with demonstrated ability to meet the ANMC competencies for the Registered Nurse
- Information Technology skills including Microsoft Office, Outlook and Patient Management Systems
- Excellent time management and organisational skills
- Evidence of a focus on and a passion for high quality patient care
