About the facility
Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
About the role
Connected Care North is a program established in 2023, operating in the northern suburbs of Geelong. It works collaboratively with the Connected Care team based in the southern region, together covering the entire Barwon Health catchment. The inter-professional team supports clients with chronic and complex health conditions, aiming to reduce hospital demand by providing comprehensive assessment, care coordination, and assistance in accessing specialist care within the community. The model of care is responsive and agile, adapting to both client and organisational needs. The team includes care coordinators, nurses, medical staff, and allied health professionals such as psychologists, diabetes educators, social workers, and pharmacists.
The Nurse Practitioner role provides clinical leadership, assessment, and treatment to clients within the program. The role supports the planning, implementation, and coordination of care aligned with the Barwon Health Connected Care model. It involves direct client care, as well as secondary consultation to support team members and external services, including general practitioners. There is also an opportunity to contribute to the evolving agile model of care, ensuring high-quality outcomes for people in the northern region living with chronic physical and behavioural health conditions, along with social challenges. The role is well supported by a registrar and medical consultants from a range of specialties.
Full-time hours are available, and part-time applicants are encouraged to apply. Nurse Practitioner candidates with extensive experience in chronic disease management and complex care are particularly welcome.
