About the facility  

Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.

As a Mental Health Nurse Practitioner , you will play a crucial role within our integrated, recovery-oriented Mental Health and Wellbeing Local Service for Greater Geelong and Queenscliffe (The Local GGQ), contributing to better outcomes of the community. 

About the role 

  • Deliver high-quality mental health assessments and short-term treatment consultations. 

  • Develop and implement comprehensive consumer care plans, ensuring seamless access to optimal care pathways and support services within our multi-disciplined service. 

  • Facilitate the transition of care plans and medication management to general practitioners and primary health service providers. 

  • Promote continuity of care and collaboration between Area Mental Health and The Local GGQ Services .
