- Temporary part-time role (32 hours per fortnight) (Long Service leave) managing a variety of strategic initiatives to support smooth transition of patient care between the hospital and primary care
About the role
Barwon Health's GP Liaison Unit (GPLU) works to assist GPs with access to hospital services and to improve communication between Barwon Health and General Practice. The primary activities of the unit include:
- Facilitating communication between the hospital and GPs in the community
- Operation of a dedicated Help desk for GPs to assist with queries, feedback, documentation follow-up and secure messaging support
- Development and implementation of referral and management guidelines and templates for GPs to assist in service demand management
- Support for the Discharge Summary program, including medical staff education
- Maintenance of local GP practice databases
- Close collaboration with Western Victoria Primary Health Network and HealthPathways, to optimise primary-tertiary care integration for the Barwon Southwest community
- Facilitating and supporting opportunities for GP integration within clinical models of care
Key responsibilities
- Lead and support quality improvement initiatives across the primary care and hospital interface, including referral pathways, discharge processes, service redesign, and digital communication projects.
- Coordinate GP service navigation and clinical handover processes, responding to enquiries, investigating issues, and facilitating effective communication between primary care providers and Barwon Health services.
- Ensure compliance with governance, documentation, and quality standards, including information management requirements, auditing activities, and continuous quality improvement initiatives.
- Promote a safe, collaborative, and professional workplace culture through adherence to Occupational Health and Safety requirements, organisational policies, and ongoing professional development.
What we’re looking for
- Current registration as a Medical Practitioner with AHPRA and Fellowship of the RACGP and/or ACRRM, with current clinical practice as a General Practitioner.
- Demonstrated leadership and influence within the primary care sector, evidenced through active involvement in professional bodies, postgraduate education, or other leadership activities.
- Advanced understanding of contemporary general practice and healthcare systems, including quality improvement, clinical governance, and primary care-hospital integration.
- Highly developed communication, stakeholder engagement, strategic thinking, and organisational skills, with the ability to build effective relationships and drive positive change across healthcare settings.
