About the facility
Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
About the Community Nursing Resource Worker (Intake) Role (open to Endorsed Enrolled Nurses- Level 2)
As a Community Nursing Resource Worker (Intake) worker you will be required to contribute to and be part of the Team to support clients and carers to achieve an optimum level of health independence through:
- Determining the most appropriate service for patients according to their presenting issue
- Provide accurate information of services, fees, eligibility and waiting times
- Ensure appropriate referral and handover information has been provided including medication orders
- Undertake an initial Home Visit Risk Screen with each patient via phone
- Work in in partnership with all relevant stakeholders
We are looking for dynamic casual staff members with:
- Experience in acute care or community nursing.
- High level IT skills
- High level communication skills
During the minimum 4 week supported orientation, you will need to be available a minimum of 3 days a week and will be required to complete competencies specific to the area. This position includes day and weekend shifts and as a casual with Community Nursing you would need to be available to work any of these. Opportunity exists to work up to 76hours a fortnight if you choose. Community Nursing casual staff often move into temporary or permanent roles with our program in the future.
