About the  role

We are seeking a passionate Community Health Nurse – Diabetes Educator to join our multidisciplinary Primary Care team based at Corio Community Health Centre. In this role, you will deliver high-quality, client-centred care that supports consumers and carers to achieve optimal health independence and improved diabetes self-management.

Working within a collaborative community setting, you will provide holistic nursing care, contribute to coordinated care planning, and support clients to build their health literacy. You will play a key role in delivering evidence-based diabetes education while contributing to service development and continuous improvement initiatives.

 

Key responsibilities

  • Provide holistic, client-centred nursing care to achieve positive clinical outcomes
  • Deliver diabetes education to support self-management and improved health literacy
  • Work collaboratively within a multidisciplinary team to coordinate care planning and delivery
  • Manage an independent clinical caseload within a community health setting
  • Contribute to service development, quality improvement, and achievement of KPIs
  • Liaise with internal and external stakeholders to ensure appropriate referrals and continuity of care 

 

What we’re looking for

  • Registered Nurse with current AHPRA registration
  • Qualification in, or working towards, Credentialed Diabetes Educator (ADEA)
  • Demonstrated experience in diabetes care and chronic disease management
  • Strong communication skills with the ability to engage diverse community members
  • Ability to work both independently and collaboratively within a multidisciplinary team
  • Commitment to continuous professional development and evidence-based practice
