About the role
As a Connected Care Remote Patient Monitoring Nurse, you will provide proactive, consumer-centred care to people living with chronic and complex health conditions through remote monitoring and care coordination. Working within a multidisciplinary team, you will assess consumers, identify early signs of clinical deterioration and coordinate timely interventions to help prevent avoidable hospital presentations. This role combines clinical expertise, technology and patient education to empower consumers to better manage their health while receiving specialist support in the community.
Key responsibilities
- Deliver comprehensive remote assessment, monitoring and care coordination for consumers with chronic and complex health conditions.
- Identify and respond to early signs of clinical deterioration, escalating care and coordinating timely interventions when required.
- Collaborate with consumers, carers and multidisciplinary healthcare providers to deliver high-quality, person-centred care.
- Contribute to quality improvement initiatives, service outcomes and continuous professional development while participating in a 7-day roster.
What we're looking for
- Current AHPRA registration as a Registered Nurse with a Bachelor of Nursing qualification.
- Demonstrated experience in chronic disease management, comprehensive clinical assessment and person-centred care planning.
- Strong communication, organisational and clinical decision-making skills, with the ability to work effectively within a multidisciplinary team.
- Experience working in community or home-based healthcare settings, with sound computer skills and a current driver's licence.
