About the facility 
Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac. 

 

The Connected Care Program aims to deliver better health outcomes and reduce hospitalisation rates by improved integration of care with existing GPs, providing specialist assessment, negotiated, comprehensive care plans and early escalation pathways for clients. The service provides a targeted and individualised service to consumers with complex chronic health or psychosocial needs at risk of recurrent ED presentations, admissions and readmissions. The service is primarily delivered in the home, with telehealth, phone or in-clinic appointments at Barwon Health facilities as required.

 

About the role

Remote Patient Monitoring (RPM) is available to consumers 7 days a week / 24 hours a day to provide consumers with chronic health conditions (e.g. CHF, COPD, Diabetes) regular contact and proactive monitoring, by phone, telehealth or face to face as required. The aim of RPM is to identify early stages of clinical deterioration and provide timely responsive access to specialist care in the community setting, in addition to supporting consumers to develop and enhanve their health literacy and self-management skils. RPM Nurses contribute to, and are apart of a community-based, multdisciplinary team to support consumer and carers acheive an optimum level of health and wellbeing through:

  • Ensuring consumers have access to quality, effective, consumer centred support based on current best practice;
  • Providing timely and responseive assessment, scheduled monitoring and escalation of care as required;
  • Collaborate effectively with internal and external key stakeholders
  • Contribute to program key pergomane indicators (KPI's)
