About the Role

Community Nursing is a dynamic and rewarding specialty that offers autonomy, variety, structured rosters, and genuine work-life balance. Based in the Geelong and Surf Coast regions, you'll join a values-driven team committed to delivering exceptional person-centred care.

Our Transition to Specialty Practice Program provides comprehensive clinical and professional support, including:

  • A structured orientation and onboarding program
  • An experienced preceptor and mentor
  • Dedicated support from a Community Nurse Educator

  • Supernumerary experiences across broader community health programs
  • Professional development leave to build specialised home-based nursing skills
  • Regular clinical debriefing, coaching, and feedback sessions

As a Community Nurse, you will undertake comprehensive assessments, develop individualised care plans, and provide clinical care to a diverse client population, including:

  • Acute and chronic wound assessment and management
  • Diabetes education and monitoring
  • Continence assessment and management
  • Hospital in the Home services
  • Post-Acute Care services
  • In-home palliative and end-of-life care

About the Facility

Community Nursing provides 24-hour, home-based nursing care across the Geelong region. Working collaboratively with hospitals, allied health professionals, general practitioners, and community organisations, our nurses support clients to remain safe, independent, and well in their own homes.

Positions Available

North Team
Based at The Hub, North Geelong
68 hours per fortnight

South Team
Based at Belmont Community Health Centre
68 hours per fortnight

You'll work a four-week rotating roster that includes day, afternoon, and weekend shifts:

  • Day Shift: 7:30am - 4:00pm
  • Afternoon Shift: 1:00pm - 9:30pm

All shifts commence and conclude at your designated office location. During your shift, you'll be provided with a fleet vehicle, mobile phone, laptop, and personal duress alarm.

About You

You are a motivated and enthusiastic Registered Nurse with a commitment to ongoing learning and professional development. You'll bring strong critical thinking skills, excellent communication abilities, and a genuine passion for providing high-quality care in the community.

Desirable:

  • Minimum three years' acute nursing experience
  • Sound clinical assessment and decision-making skills
  • A commitment to person-centred care
  • Values and behaviours that align with Barwon Health's values
