About the facility
Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
About the role

If so, then this permanent ongoing opportunity with the South (Belmont) Community Nursing Team at 60 hours per fortnight may be the role for you.
In this role you will be responsible for the provision of patient focused holistic nursing care to those assigned to you in order to achieve desired clinical outcomes. You will have highly developed interpersonal skills as well as sound communication skills both verbal and written.
The successful applicant will need to have current/recent acute or community health experience. Experience & knowledge in the areas of wound care, palliative care, diabetes and continence management and community health care principles are desirable.
About Community Nursing
Barwon Health Community Nursing is the largest non-metropolitan based service in Victoria; Community Nursing is a government funded program which provides home nursing services.
Community Nursing consists of three integrated teams based at Barwon Health’s Newcomb, Belmont and McKellar Centre
About the program
Our program supports clients and carers in their home environment to achieve an optimum level of health independence through the development and delivery of a high quality, cost effective, locally relevant Community Nursing service. We provide Palliative Care (CPC), Post-Acute Care (PAC), Commonwealth Home Supports (CHSP), Hospital in the Home (HITH), National Disablity Insurance Scheme (NDIS) & Home and Community Care (HACC) nursing services.
