Community Nurse (Level 1 or 2),Community Health, Rehabilitation and Palliative Care

Key responsibilities 

• Deliver high-quality, patient-centred care aligned with evidence-based practice, professional standards, and Barwon Health policies.
• Ensure safety, quality, and compliance with national standards, including participation in audits, incident reporting, and continuous improvement initiatives.
• Collaborate effectively within a multidisciplinary team to support coordinated care and contribute to a positive team culture.
• Provide patient-centred care using evidence-based practice, aligned with professional standards and Barwon Health policies.
• Maintain professional and operational standards through ongoing education, accurate documentation, and compliance with OHS and organisational requirements.

What we’re looking for 

• Degree level qualification with a recognised educational institution in Nursing
• Registration with AHPRA as a Registered Nurse Division 1 
• Current Working With Children Check
• Current Police Check 
• Current Victorian Driver’s Licence
• Current and valid NDIS Worker Screening clearance 
• Completion of ONLINE - QUM National Standard Medication Chart Training 
• Completion of ONLINE - QUM Medication Safety Training 
• Information Technology skills including Microsoft Office, Outlook and Patient Management Systems 
• Minimum 2 year post graduate nursing experience in an acute hospital setting
• Previous Community Nursing experience
