About the facility 
Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac. 
 

About the Newcomb Primary Care Team 

The Primary Care team is a community-based team of multidisciplinary health professionals from allied health and nursing backgrounds that supports consumers and carers to achieve an optimum level of health independence through the provision of client-focused, holistic care to achieve the individuals desired health outcomes. This position is based at the Newcomb Community Health Centre. 


About the role

As part of Public Health and Primary Care Unit deliver services to the multi-disciplinary Primary Care program area. Community Health Nurses are responsible for the continuous improvement and delivery of high-quality, cost-effective community health nursing services.

All activities of the position will reflect current best practice standards for nursing and support consumers and carers to achieve an optimum level of health independence through:

  • Provision of client focused holistic nursing care to achieve desired clinical outcomes
  • To work with community members from a broad range of backgrounds to ensure clients are engaged
  • and in their health care and are improving their health literacy
  • Actively participate in comprehensive, co-ordinated and integrated multidisciplinary services.
  • Contribute to the development and delivery of high-quality cost-effective community-based Community Health Nursing service
  • Contribute to meeting the primary care program and nursing key performance indicators (KPI’s).
  • Provision of holistic screening of referrals and enquiries, to ensure consumers achieve access to the correct services and priority for care according to their needs

 

Primary Care (PC) Intake is an entry point for referrals to the PC service and connects consumers with the most appropriate services at Barwon Health to meet their individual needs. The intake worker has a strong overview of services across Barwon Health and can direct consumers to the most appropriate. They are responsible for the provision of holistic screening of referrals and enquiries in order to ensure consumers have access to the correct services and priority for care.

Working within their scope of practice, they will identify additional services of benefit to the clients and provide a range of information to consumers about services, fees, eligibility criteria, and waiting times.

The intake worker will liaise closely with the PC team to confirm service prioritisation or escalate care to ensure a safe and high quality experience for our consumers. This role will also work in collaboration with Information & Access Service and site Clinical and Administrative staff.
