Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
About the role
Barwon Health is recruiting a Community Health Nurse to offer victims of sexual assault and/or family violence and their family’s access to holistic health care. Working within the Barwon Multidisciplinary Centre, you will work collaboratively with key service providers including Sexual Assault & Family Violence Centre counsellor advocates and case managers, Victoria Police, and Child Protection to improve the health care of vulnerable individuals.
The Multidisciplinary Centre (MDC) Community Health Nursing (CHN) service is a state-wide initiative providing a service on an in-reach and outreach basis from each MDC. Working from a trauma informed approach, the CHN will undertake initial needs identification and provide direct nursing services to victims/survivors of sexual assault and/or family violence.
The MDC nurse will identify client health needs via a holistic health assessment, develop and implement client centred care plans, provide health education, increase health literacy, and assist clients to engage in health care. Excellent clinical skills will ensure the delivery of best-practice health care services to sexual assault and family violence survivors with a particular focus on assessment and care coordination.
This position will be based at the Barwon Multidisciplinary Centre, located in Geelong West. The Barwon MDC CHN will:
- Advise victim/survivors on the health implications of sexual assault
- Perform health assessments and facilitate their access to other health services as appropriate
- Provide individual health and wellbeing support, advocacy and nurse-specific counselling, educate various stakeholders about health issues affected MDC clients
- Establish effective local health care networks with the objective of improving the care of victim/survivors of sexual assault and family violence, and contribute to the effective operations of the state-wide MDC nursing network
- Work within the MDC CHN team to triage and prioritise referrals
In the role of MDC Community Health Nurse, you will demonstrate the ability to work autonomously, demonstrating the ability to prioritise tasks. You will possess excellent clinical and communication skills with a particular interest in contributing to the development of an innovative model of care and drive initiatives, support relationship development and collaboration.
