About the role
As the Clinical Nurse Consultant (CNC) will act as a clinical nursing leader in the delivery of sub-acute nursing, providing care, education, mentorship, secondary consultation and clinical advice at an expert level to the nursing staff within the Subacute at Home service. As the CNC will provide holistic nursing care to people who have complex needs and ensure the broader Subacute at Home team’s care is supported by clinical oversight. Care is provided within the consumer’s home with a strong emphasis on empowerment utilising dignity of risk principles. You will provide a clinical resource, clinical advisory/developmental role and lead the speciality team of nurses.
Key responsibilities
- Lead and coordinate the day-to-day delivery of clinical nursing care within the Sub-Acute at Home Program, ensuring appropriate staff allocation and skill mix.
- Support and develop nursing staff to work to full scope of practice through clinical guidance, education, and fostering evidence-based practice.
- Ensure compliance with National Safety and Quality Standards, contributing to continuous improvement, research, and safe care delivery.
- Collaborate effectively with multidisciplinary teams to support staff performance, professional development, and high-quality patient outcomes.
What we’re looking for
- Current registration as a Registered Nurse with Australian Health Practitioner Regulation Agency
- Post graduate qualifications in relevant field or working towards completion of a qualification
- Extensive experience providing sub-acute clinical care in a community or at home setting
- Demonstrated expert skills and knowledge in completing patient assessments and care planning Practices safely, applies advanced nursing skills and is a role model Evidence of ongoing self-development Current Victorian Driving Licence (or equivalent)
