Clinical Nurse Consultant, Subacute

Key responsibilities 

• Lead and coordinate the day-to-day delivery of clinical nursing care within the Sub-Acute at Home Program, ensuring appropriate staff allocation and skill mix.
• Support and develop nursing staff to work to full scope of practice through clinical guidance, education, and fostering evidence-based practice.
• Ensure compliance with National Safety and Quality Standards, contributing to continuous improvement, research, and safe care delivery.
• Collaborate effectively with multidisciplinary teams to support staff performance, professional development, and high-quality patient outcomes.

What we’re looking for 

• Current registration as a Registered Nurse with Australian Health Practitioner Regulation Agency
• Post graduate qualifications in relevant field or working towards completion of a qualification
• Extensive experience providing sub-acute clinical care in a community or at home setting
• Demonstrated expert skills and knowledge in completing patient assessments and care planning  Practices safely, applies advanced nursing skills and is a role model  Evidence of ongoing self-development  Current Victorian Driving Licence (or equivalent)
