About the role

The Post Acute Care Clinical Administration Officer provides timely, client-focused administrative support to facilitate smooth service delivery across the program. The role contributes to efficient referral processing, document management and general administrative tasks that support effective patient flow and quality outcomes. The role may be required to support multiple Barwon Health clinical services depending on service delivery needs. The position will operate according to relevant legislation, Barwon Health policies, programs, and department guidelines.

The successful applicant will be well supported by the Post Acute Care Clinicians, however it would be advantageous for the successful applicant to understand and be able to interpret medical terminology.

The Post Acute Care Program aims to assist clients recently discharged from public hospitals requiring short-term, community-based support to assist them to recuperate in the community and ensure safe and timely discharge. The service has recently undergone a model of care review and is now offering a new and exciting program to meet the needs of our clients as they transition back home.

 

Key Responsibilities

  • responsible for client focused administrative support for all incoming referrals received by the Post Acute Care Department
  • a central point of contact for clients, families/carers, internal and external service providers who require access to the Post Acute Care program
  • responsible for supporting the Post Acute Care Clinicians in managing incoming and outgoing referrals across a number of data platforms
  • required to be tech savvy working across a number of digital systems, data platforms, and service coordination tools

 

What we’re looking for

  • previous health administration experience
  • demonstrated ability to communicate with people at all levels and varying communication styles
  • the ability to work in collaboratively within a small team
  • a proven ability to multitask, manage competing priorities and problem solve
  • proven computer literacy including the use of Microsoft Office applications (word, Excel, Outlook, TEAMS) and ability to learn specialist programs
