About the role
Join Barwon Health’s Subacute at Home Program as an Allied Health Assistant and make a meaningful difference by supporting patients to achieve greater independence and wellbeing in their own homes.
Working across the Geriatric Evaluation and Management (GEM) and Rehabilitation streams, you will provide high-quality, patient-focused care under the direction and supervision of Allied Health Professionals. You’ll work as part of a multidisciplinary team, supporting patients through their rehabilitation journey while contributing to their goals and clinical outcomes.
This is a great opportunity for an experienced Allied Health Assistant who enjoys working with people, values collaborative care and is looking to broaden their skills within a dynamic community-based service.
Key responsibilities
- Provide patient-focused Allied Health interventions in patients’ homes under the direction of an Allied Health Professional.
- Support patients across the GEM and Rehabilitation streams to achieve their health, wellbeing and independence goals.
- Manage an allocated caseload and work collaboratively with Allied Health Professionals and the broader multidisciplinary team.
- Complete clinical documentation and other administrative tasks accurately and within required timeframes.
What we're looking for
- Certificate IV or above in Allied Health Assistance, or an equivalent qualification.
- Demonstrated experience and skills appropriate to working as an Allied Health Assistant in a clinical setting.
- Experience supporting patients with exercise, rehabilitation or functional activities is highly desirable.
- Strong communication and interpersonal skills, with the ability to build positive relationships with patients, families and colleagues.
- Ability to work independently while also contributing effectively to a multidisciplinary team.
