About the facility
Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
Barwon Health’s residential TCP is provided at Alan David Lodge in Charlemont, situated on the Surf Coast Highway and only a short drive from central Geelong.
Alan David Lodge provides 108 high-level care beds, with 32 beds allocated to Transition Care. The prgram provides support for older adults following hospital discharge, with tailored care plans and ongoing coordination. Care is delivered by a multidisciplinary team including nurses, allied health professionals, and care coordinators.
About the role
As the Allied Health Assistant, you will contribute to the care of Barwon Health TCP clients by delivering client-centred, evidence-based services under the supervision of the Allied Health and Care Coordination teams. You will also support nursing staff with direct care functions and assist with non-clinical duties to ensure seamless service delivery within the Transition Care Program.
