About the role

As an assessment clinician you'll work independently within a multidisciplinary team to deliver the Aged Care Assessment Program in the VIC 3 region. You're primary responsibilities will include conducting comprehensive assessments across physical, medical, psychological, cultural, and social care dimensions, and offering consumers appropriate service options. You'll collaborate with clients to assess their needs, goals, and preferences, focusing on wellness and reablement to promote independence. You'll provide information and refer clients to suitable services, including residential care, respite, Support at Home, and other relevant programs such as DVA, Mental Health, or NDIS.

 

Key Responsibilities

  • Conduct Aged Care Need assessments of the restorative, physical, medical, psychological, cultural and social dimensions of care and provide consumers with a choice of appropriate services 
  • Work with the client to assess their needs, goals and preferences focusing on a wellness and reablement approach to build their independence 
  • Provide information and refer clients to services that are appropriate and available to meet their needs and preferences. This may include facilitating access to residential care, residential respite or community packaged care, plus broader community services such as DVA, Regional Assessment Service (RAS), CHSP (Commonwealth Home Support Program) Mental Health or NDIS. 

 

What we’re looking for

  • Tertiary qualification within relevant health discipline as per ACAP Guidelines 
  • Current practicing certificate/registration/accreditation as appropriate
  • Holds and maintains a current Victorian driver’slicence 
  • Demonstrated experience in comprehensive assessment skills to determine care needs of complex frail older clients
  • Highly developed written and verbal skills with clients, carers, colleagues and other professionals
  • Competent use of computer programs and technologies • Ability to utilise initiative and professional judgement, clinical knowledge and expertise to function as an independent practitioner in a multidisciplinary assessment service
  • Demonstrated ability to work autonomously, as well as part of a team
  • A sound knowledge of the health and aged care service system across allsettings
  • Ability to work effectively as part of a team in a fast-changing environment, to work productively with limited supervision and to work to deadlines
  • Currently hold and maintain unrestricted registration with APHRA or AASW and do not have any matters pending which may result in restrictions
